Your Responsibilities

You agree to provide accurate information during intake and to use this site only for its intended purpose. Misuse of this site, including submitting false information, may result in termination of access.

Payment and Billing

Pricing for programs is displayed on our Programs page and is subject to change. Billing terms will be presented at checkout.

Prescriptions

Any prescription is issued solely at the clinical judgment of a licensed provider following your intake and consultation. We do not guarantee that any specific medication will be prescribed.
